That’s why we provide free resources to help you understand and navigate care at home.
Quarterly Nurse Assessment Report Card
Every family we serve receives a free nurse visit every quarter.
During these visits, a board-certified nurse evaluates:
- Vitals & overall health status
- Mobility & fall-risk assessment
- Medication safety & adherence
- Nutritional needs
- Caregiver support check-in
We put all of this into a simple, easy-to-read “Care Report Card” that shows your loved one’s progress and areas of focus.
What Transitional Care Really Means
Transitional care is about helping people recover at home after being in the hospital — so they can heal safely, avoid unnecessary readmissions, and regain independence.
Our approach includes:
- Day 1–7: Home safety setup, medication review, discharge instructions explained, coordination with doctors.
- Day 8–14: Daily living support, physical therapy reminders, emotional support check-in.
- Day 15–21: Progress monitoring, symptom tracking, caregiver education.
- Day 22–30: Preventive planning, long-term care strategies, nurse follow-up.
