Laxmi’s story

Every great mission begins with a personal journey. For Laxmi Chhetri, BSN, RN-BC, the path to founding Dignity Transition Care started humbly and grew into a calling.

Laxmi began his career in healthcare at the ground level, working as a housekeeper and clinical assistant at Ascension. These early years taught him not only the importance of safety and cleanliness in healing environments, but also the profound impact that small acts of compassion can have on patients and their families.

Determined to expand his ability to serve, Laxmi pursued nursing, earning his Associate Degree in Nursing (ADN) from Austin Community College and continuing his education at the University of Texas at Arlington, where he is completing his Master of Science in Nursing (MSN) as a Family Nurse Practitioner (FNP).

Over more than a decade with Ascension Health, Laxmi advanced from Fall Prevention Nurse to Clinical Nurse Supervisor and ultimately Charge Nurse and Co-Chair of Shared Governance. In each role, he was known for championing patient safety, mentoring staff, and ensuring that care remained compassionate and culturally sensitive. His work earned him the DAISY Award for excellence in nursing (2023) and multiple Nurse of the Year nominations.

Yet, Laxmi saw a recurring challenge: patients often thrived in the hospital but struggled once they returned home. Families felt unprepared, recovery stalled, and preventable readmissions occurred. Out of this insight, he founded Dignity Transition Care in 2025 to bridge the critical gap between hospital discharge and in-home independence.

Today, Laxmi leads a team dedicated to transitional care, chronic disease support, and palliative services. His guiding principle is simple yet profound: “Service to humankind is service to God.” Through Dignity Transition Care, he has turned that belief into a model of care that blends clinical expertise with compassion, ensuring every patient feels safe, respected, and empowered to heal at home.
